** A Student Midwife is defined as a person pursuing a recognised programme of study leading to qualification as a midwife, approved by the appropriate regulatory authority in the country, in line with the ICM definition of a midwife.
The official stamp of the training institution will be required as verification and should be sent to the congress secretariat.

Cancellation registration
Notification of cancellation and refund requests must be submitted before March 1, 2008 in writing to Concorde, the organising secretariat. The fee for cancellations received on or before March 1 2008 is Pounds 50. For cancellations received after March 1 2008 no refunds will be given. All balances must be settled upon registration at the congress.

Disclaimer
For reasons beyond their control, the ICM 28th Triennial Congress and/or its agent Concorde and/or its agents have the right to immediately alter or cancel the congress or any of the arrangements, timetables, plans or other items relating directly or indirectly to the ICM 28th Triennial Congress.
The participants shall not be entitled to any compensation for damages that result from such alteration or cancellation. Furthermore, with the exception of any wilful damage or gross negligence committed by the ICM 28th Triennial Congress and/or its agent Concorde and/or its agents, neither the ICM 28th Triennial Congress nor its agent Concorde and/or its agents shall at any time be liable for any direct or indirect damage suffered by the participants, including consequential and immaterial damage, caused by failure to comply with any provision of this document.
It is highly recommended that all participants carry proper travel and health insurance.

After May 15, 2008, we request you to register at the congress on-site registration desk at the Scottish Exhibition and Conference Centre (SECC). After this date an on-site registration form (PDF-format) can be found on the website www.midwives2008.org.

Please print this form, complete it and bring it to the on-site registration desk.

Registration via regular mail

Please fill out the enclosed registration form and send it to Concorde, the ICM organising secretariat at: fax +44 (0)20 7117 4562. This form should be completed by either typing or writing in block capitals. Please note that the form is to be used for the registration of one participant only and his or her accompanying person(s). Please read the registration and payment information carefully before completing your registration.

Confirmation

Your registration will be confirmed by Concorde once your registration has been received. Please contact Concorde if you have not received a confirmation letter six weeks after your registration. A final confirmation will be mailed to you four weeks prior to the Congress. This confirmation letter must be presented upon arrival at the pre-registration desk if the account has been settled or at the non-paid desk if a balance is still due. If you do not have a confirmation letter, you are kindly requested to bring your passport or other form of identification with you.

Group registration

If you are interested in group registration (10 or more people), please contact Concorde at registration@midwives2008.org for the necessary forms. Please note all groups must be pre-registered and paid for before the stated deadlines. Registration of new groups on-site is not possible; all new registrations will be handled on an individual basis.
